• The GP data controller for the practice should enter their full name followed by their email address into the boxes provided below and click the ‘Begin Signing’ button.


• You will shortly be presented with an electronic access agreement document for signing. You will be prompted to complete a number of fields, please note most fields are mandatory. Once the information has been entered and the document has been signed, click the yellow ‘Finish’ button to complete the approval process. You will receive a copy of the signed document via email for your records.
